I here by release, discharge, & hold Real Ink Tattoos, its employees, & agents from any & all claims, cause of action suits & liabilities of any nature or kind arising out of my request for the application of a tattoo or body piercing. I understand there is an element of risk involved in the application of a tattoo or body piercing & I understand & assume all risks connected with this procedure. No promises or representations of any kind have been made to induce me or undergo this procedure. I understand if I forfeit my tattoo session, for ANY reason there will be a $60 set up fee if & when I return to finish my tattoo. Deposits are NONREFUNDABLE. On reliance to this executed release, which I have read & understand, Real Ink Tattoos has agreed to perform the application of this procedure which I have requested & instructed them to perform. I further certify that the risks have been explained and understood between all parties & I completely & forever hold Real Ink Tattoos unaccountable for any liability or responsibility arising from this procedure, the result thereof, any problems or claims associated with the application of the tattoo or body piercing. I further attest that I am at least 18 years of age or I am a Parent or Legal Guardian signing in consent.
